چکیده

we discuss here an uncommon condition of neurogenic hypotension in the context of immunoglobulin light chain (amyloid light-chain) amyloidosis. th e most serious feature was autonomic nervous system impairment, mainly characterized by severe refractory orthostatic hypotension, which became progressively invalidating, forcing the patient to bed. moreover, since the systemic involvement of the disease, the patient presented also diarrhea, dysphagia, asthenia, peripheral edema because of gastrointestinal, and kidney dysfunction. eventually, the massive myocardial depression and infi ltration led to a fatal outcome.

Epiploic appendagitis is one of the self-limiting rare causes of acute abdominal pain that is caused by torsion or accidental necrosis of one of the epiploic appendix of the colon. Droxidopa for neurogenic orthostatic hypotension

OBJECTIVE To determine whether droxidopa, an oral norepinephrine precursor, improves symptomatic neurogenic orthostatic hypotension (nOH). METHODS Patients with symptomatic nOH due to Parkinson disease, multiple system atrophy, pure autonomic failure, or nondiabetic autonomic neuropathy underwent open-label droxidopa dose optimization (100-600 mg 3 times daily), followed, in responders, by 7-...

Neurogenic orthostatic hypotension: a pathophysiological approach.

Orthostatic hypotension (OH) is defined as a persistent, consistent, orthostatic fall in systolic blood pressure of 20 mm Hg or diastolic pressure of 10 mm Hg by 3 minutes of standing up.1 Acute, unexpected, episodic falls in blood pressure while standing, as in neurocardiogenic syncope, do not satisfy criteria for OH. Medical records for 0.4% of all hospitalizations in the United States includ...

Pathophysiology, diagnosis, and treatment of orthostatic hypotension and vasovagal syncope.

Orthostatic hypotension (OH) occurs in 0.5% of individuals and as many as 7-17% of patients in acute care settings. Moreover, OH may be more prevalent in the elderly due to the increased use of vasoactive medications and the concomitant decrease in physiologic function, such as baroreceptor sensitivity. OH may result in the genesis of a presyncopal state or result in syncope. OH is defined as a...
